目的 探讨皮下放置负压引流管对预防肥胖患者急诊剖腹探查术后切口感染的作用。
方法 选取具有可比性的急诊行剖腹探查术的肥胖患者240例,切口类别为II类及III类切口,随机分为实验组及对照组各120例,缝合切口时实验组皮下放置负压引流管,对照组常规缝合,观察两组患者切口甲级愈合情况、切口感染率、二期缝合情况及住院时间。
结果 切口甲级愈合实验组为110例,对照组为90例,实验组明显高于对照组(P<0.01);切口感染实验组3例,对照组11例,实验组明显低于对照组(P<0.05);二期缝合实验组2例,对照组9例,实验组明显低于对照组(P<0.05);住院时间实验组为7.72±3.144天,对照组为12.28±4.512天,实验组明显低于对照组(P<0.01)。
结论 肥胖患者急诊行剖腹探查术II类及III类切口缝合切口时皮下放置负压引流管能提高切口甲级愈合率,降低切口感染率,降低二期缝合率及缩短患者住院时间,值得临床进一步推广应用。 |

Objective To investigate the effect of subcutaneous placement of negative pressure drainage tube on prevention of incision infection after exploratory laparotomy in obese patients.
Methods Select a comparable emergency laparotomy in 240 cases of obese patients, incision type both class II and class III incision, were randomly divided into experimental group and control group 120 cases, incision suture in experimental group subcutaneous negative pressure drainage tube, the control group routine suture, observe two groups of patients with wound healing. Two, the rate of incision infection, suture and hospitalization.
Results Wound healing in experimental group was 110 cases, 90 cases in the control group, the experimental group was significantly higher than the control group (P<0.01); 3 cases of wound infection in the experimental group, 11 cases in the control group, the experimental group was significantly lower than the control group (P<0.05); two suture in 2 cases in the experimental group, 9 cases in the control group, the experimental group was significantly lower than that of the control group (P<0.05); hospitalization time of the experimental group was 7.72 + 3.144 days, the control group was 12.28 + 4.512 days, the experimental group was significantly lower than the control group (P<0.01).
Conclusions Obese patients with emergency laparotomy II class and III class incision suture incision subcutaneous negative pressure drainage tube can improve the healing rate of incisions, decrease the infection rate of incision, suture two reduction rate and shorten the hospitalization time, is worthy of further clinical application. |
